When you change your medical insurance,
to advise us of the change. The updated information will be loaded in
our system and save time when you arrive for your next appointment. If
you have a scanner or a digital camera, it would be helpful to us if
you would include an image of both sides of your new card.
When you move,
to advise us of your new address. This link may also be used to notify
us of other changes of demographic status such as birth, death,
marriage, divorce, etc.
when you need extensive assistance with dealing with billing issues.

Multicare periodically mails preventive health letters to our patients to tell them about tests that are advised. Most of these letters are sent automatically, based on information in our computerized billing and scheduling systems. Some letters are initiated by your healthcare provider. Our recall letters are mailed on “Preventive Health Services” letterhead.
We try to target recall letters correctly, but admit that we do not always succeed.
